Substance use disorders (SUDs) are chronic, treatable conditions that commonly present alongside mental health disorders, medical comorbidities, and social risk factors (Volkow & Blanco, 2023). Fragmented treatment systems contribute to gaps in identification, engagement, and continuity of care. Emerging research demonstrates that whole-person, integrated care models delivered through interdisciplinary teams and embedded within primary and behavioral health settings improve access, treatment retention, and health outcomes while reducing stigma (Farhoudian, et. al, 2022). Effective care models combine medication treatment, behavioral health services, care coordination, peer support, and strategies to address social determinants of health (AIMS, 2024). This presentation reviews the evidence base for integrated SUD care and provides practical approaches to implementing coordinated, person-centered treatment. Attendees will gain tools to strengthen collaboration when managing co-occurring conditions and deliver comprehensive care that supports long term recovery.
